Afamelanotide
Afamelanotide is a lab-made compound that helps the skin better handle sunlight. It gently increases skin color, allowing people who are very light-sensitive to spend more time outdoors comfortably. It also supports the skin’s natural protection system against sun damage.
Description
- Afamelanotide, is a synthetic analog of the natural alpha-melanocyte-stimulating hormone (α-MSH).
- Developed to activate the melanocortin-1 receptor (MC1R) to increase melanin production and provide photoprotection.
- Commercially approved under the brand name Scenesse® for the treatment of erythropoietic protoporphyria (EPP).
How It Works
- Binds selectively to MC1R receptors on melanocytes, stimulating eumelanin synthesis and darkening skin pigment.
- Eumelanin absorbs ultraviolet radiation and reduces oxidative stress within skin cells.
- Also exhibits mild anti-inflammatory and antioxidant effects through melanocortin signaling pathways.
Documented Effects
- Clinically shown to increase tolerance to sunlight and reduce phototoxic pain in patients with EPP.
- Produces gradual, even skin pigmentation without UV exposure.
- Demonstrates protection against oxidative and UV-induced DNA damage in skin tissue studies.
- Explored as an adjunct for vitiligo, photoaging, and other pigment disorders.
Theoretical Effects
- Potential applications in sunless tanning and cosmetic photoprotection.
- May reduce inflammation and free radical generation in skin barrier research.
- Synergistic with PT-141 (Bremelanotide) for systemic melanocortin pathway studies, though their targets differ.
Pharmacokinetics
- Half-life: approximately 30–50 minutes after subcutaneous injection.
- Time to peak concentration: around 1–2 hours following administration.
- Steady-state: reached within 2–3 days when delivered by sustained-release implant.
Duration of Effects
- Each implant provides a controlled release lasting about 60 days.
- Skin darkening and light-tolerance improvements can remain noticeable for several weeks after completion.
- Effects fade naturally within 2–3 months once treatment stops.
Reconstitution
- Use bacteriostatic water for reconstitution.
- Example vial: 10 mg Afamelanotide
- Choose reconstitution volume based on dosing convenience:
- 1 mL water → 10 mg/mL → 1 mg per 10 units
- 2 mL water → 5 mg/mL → 500 mcg per 10 units
- 4 mL water → 2.5 mg/mL → 250 mcg per 10 units
- Gently swirl until clear; avoid foaming.
- Refrigerate reconstituted solution (2–8 °C). Use within ~30 days for best stability.
Injectable Dosage Overview (Clinical and Research Reference)
Typical Clinical Dose
- Approved protocol (Scenesse®): one 16 mg subcutaneous (SC) implant every 2 months for EPP patients.
- Provides steady systemic release for approximately 60 days.
Common Research Range
- Typical injectable range: 250–1000 mcg SC daily or every other day for pigmentation or UV-tolerance studies.
- Topical or transdermal formulations have been explored for cosmetic and dermatologic research.
Maximum Dose (Clinical Context)
- Regulated therapeutic exposure equals one 16 mg implant every 60 days.
- Frequent or higher dosing not supported by safety data and may cause excessive pigmentation or nausea.
Standard Cycle Length
- Implant course: continuous 2-month release; can be repeated cyclically under supervision.
- Research protocols: 4–8 weeks of daily or alternate-day injections followed by rest period.
Clinical Notes
- Implant is inserted under the skin of the abdomen by a healthcare provider.
- Injections, when used, should alternate sites to prevent irritation.
- No tapering required; pigment gradually fades after treatment ends.
- Stacking with other melanocortin peptides may enhance effects but increases side-effect likelihood.
- Stable for about 30 days when refrigerated (2–8°C). Protect from light and heat. Avoid freezing.
Safety Notes
- Generally well tolerated in clinical use; most effects are mild—nausea, flushing, or darkening of moles/freckles.
- No hormonal or carcinogenic effects detected in long-term follow-up of approved EPP therapy.
- Ensure purity and authenticity; counterfeit or unregulated peptides vary in potency.
- Approved medically for EPP; other uses remain experimental.
